Dischi, I solved this with a work around.
The mplayer dvd://1 flag causes mplayer to automatically play the whole movie, regardless of how the titles are arranged. So the only problem is how to get Freevo to pass the file name directly to mplayer, without the unnecessary additional screen.
I solved this by changing the iso file extension on my DVD files to something Freevo wouldn't recognize (I used dso) and then adding this extension to the VIDEO_MPLAYER_SUFFIX list and the MPLAYER_ARGS list. The arguments for this new extension include dvd://1 and -dvd-device.
Now when I select a movie from my DVD library in Freevo it immediately plays the whole movie, which is what I want.
It would be nice in the long run to have a configuration setting that told Freevo whether to just play the iso or show the title structure.

E.g. sometimes the movie is not title 1, sometimes there are more than one important titles (e.g. tv shows on DVD, more than one ep per disc).
