There could be several problems that you have.
1) Are you sure that your tv card is supported by Linux (Guess that your
using the bttv driver) Try googling for linux <tv-card> driver
2) Are you sure that your tv card is not broken? Could test on a Windows
machine.
3) The width and height I gave you are for PAL land, may be you need a
different sizes for NTSC land.
4) You can set the frequency directly from mencoder command line, take
out the channel and chanlist and add the frequency option, check man
mplayer for the correct syntax OR you can tune it with another tool,
v4lctl or my v4ltune from http://www.linuxowl.com/v4ltest.html.
