I use Hiper's 1st generation media centre case, which isn't wonderfully 
quiet, but should fit a Pentium M..

However they also now have a 2nd generation case:

This looks like it ticks your boxes, VFD, optional IR module.
And with the right customised back panel / motherboard you get DVI and 
component video output at the back. It is Hi-Fi separates width, so not 
small in width, but is very slim, the size of a thin height DVD player.

No idea on how it noisy it will be, with such a small case there will be 
some fan noise, but the core duo 2's run pretty quiet.

Not managed to find many places selling it in Europe though :(
