The remind plug-in can do this, it is a small plug-in and so is easy to follow. To be honest I've not looked into this except when writing oneclick. These are both main menu plug-ins and have not tried doing this for a child-menu. A quick guess is that the code to display an icon needs to be in a skin plug-in.
