I have seen both happen. Admittedly, not as often as in the pre-XP days, though.
I have also (considerably more rarely), seen equivalent strange behaviour on Linux systems. I have no particular theories - it didn't happen often enough to significantly impact on productivity, but then I've learnt to "save, save, save...", and also have daily backups.
