Harm can only be defined.

Well, true. But that's partly the point of the harm reduction approach. Regardless of how you define harm, reduction is the objective, not elimination. We could convert it to benefit reduction and it would still be a good practice ... it's simply a restatement of "everything in moderation" ... or, perhaps even, "don't linearize the non-linear".
