Well if you want a full-on VoIP/PBX phone server, there's Asterisk
http://www.asterisk.org. I've just heard of it from some of the
podcasts that use it, apparently its mostly used on Debian.
I haven't used open source VoIP myself before, just Skype. Google's
VoIP library is open source at
http://code.google.com/apis/talk/index.html and Kopete has beta
support for it.
