This is a massive commit to move information from the FAQ and User Guide to the Operations Guide. I did a bit of editing along the way but the content is pretty much unchanged from what was in the 4.1 docs. For the sake of sanity, I'd like to treat this as a relocation commit; I can cut in small comments but if you see something that needs significant rewrites or additions, I'd like to do that as a separate effort.
