Thank you for the help, none if it worked. I had to re apply the master file
associations in IIS to all the domains and then it worked. I call that a
weird bug.

Right, you've probably installed the latest NT Service Pack.

I've got around this by changing the .htm to .cfm on all pages (or just
leaving it off).

You'll need to modify formurl2attributes and get rid of the .htm
