I downloaded the code for the simplefuse and want to play with it locally
but it says I need custom tag FORMURL2ATTRIBUTES.  Where can I get this?

I don't think you need the book to grasp the basic concepts of
the fusebox methodology.  The main elements that come to mind:

1) you recursively call the same file, which consists of large
chunk of logic code (cfswitch or even cfif).

2) files are modularized, broken up into three distinct groups:
        - dsp_ display files, used to display output
        - act_ action files, used to perform actions
        - qry_ query files, used for queries

I am by no means the leading authority on this, but that's the
general idea.  There's a really simple sample application at:
