In you app_globals.cfm change to the following

<cfparam name="DSN" default="Equipment">
<cfif NOT isDefined('attributes.fuseaction')>
     <cfmodule template="formurl2attributes.cfm">
</cfif>

or just <cf_fuseaction>

however it must be call as a custom tag/module not just "included".
