I sometimes get this when working with a pdf that has corrupt / fp unexpected crop margins. I would try to take the original pdf and recreated it - I normally just use the file/print to adobe pdf and create the correct page size. You may have to change the trim / bleed settings afterwards - make sure that is done before creating any fp elements.
