I frequently run into this problem. Both with rotation and cropping. The way to 'fix' the pdf is to print it to a new pdf. Doing this creates a new pdf that is the proper size and rotation.

For example, client submits an 8.5x11 file with 10up business cards and it's rotated for whatever reason. I rotate it to what I want, crop it down, then in Page Setup I create a custom page size of 3.75x2.25 (business card plus bleed) and print to pdf using that size.
