I'll pipe in with my 2 cents here.

We've been utilizing the demo versions of the products to send our clients
examples of how direct imaging could be used.  It gets alot of WOW, but
client's don't want to pay the extra per piece price that you should be
charging for this service.  We had only one customer bite so far, and we
were able to pull off what they wanted without using direct imaging.  We
just used a "clever font" to do what they wanted accomplished.

I'd say that direct imaging is following a normal product Diffusion (The
process by which a new idea or new product is accepted by the market)

# Innovators – venturesome, educated, multiple info sources;
# Early adopters – social leaders, popular, educated;
# Early majority – deliberate, many informal social contacts;
# Late majority – skeptical, traditional, lower socio-economic status;
# Laggards – neighbors and friends are main info sources, fear of debt.

Right now, we are most likely crossing between the Innovators and the
early adopters.  It's just a matter of time before the early majority
starts to adopt the new idea.

However, I would also say that price point is also a big factor here. 
Whether I'm using custom Photoshop javascript (or the built in Photoshop
Variable Data stuff), Direct Smile, etc, there's more time/cost in setup,
production, larger file sizes, longer rip time, etc.  Although clients are
wowed by this, and think their customers will be wowed, they decide it is
not that "cool" when they see extra price added to the per piece.

I think that as a community we need to find a way to bridge this gap.  We
need to figure out ways to lower the cost of entry for a client and sell
it as an add-on service vs. a per piece price.  This should help speed up
the adoption rate of Direct Imaging in my opinion.
