I've written much of this in various other places at various times but an invitation from a colleague doing a book in these areas spurred me to take another look and I realized in doing so that the issues haven't changed all that much over the last decade or so and if anythng the "innovation" mantra has gotten even stronger and the shift of attention and resources away from bottom up processes (and including now in LDC's) has gotten even stronger.
