Just use a regular checkbox and in the name specify it as an array

<input type='checkbox' name='test[]' value='1' />
<input type='checkbox' name='test[]' value='2' />
<input type='checkbox' name='test[]' value='3' />
<input type='checkbox' name='test[]' value='4' />


Then on the PHP side just refer to $_POST['test'] as an array/.
