I would prefer to have the cache outside/above the document root, but is the .htaccess able to point to files outside of this?
No, .htaccess cannot serve files outside the web root. Just curious, but why would you want to serve cache files to the client/front-end anyways? Caching is a server-side function that makes processing on the server faster so pages load quicker.
