Luis seems to have come up with something significant. 

Once I enabled Multiple Users and turned it on, every time I restarted 
the Startup Items executed fine. Previously the control panel and 
extension were disabled. There is now just one User setup, the Owner.

If I turn Multiple Users off in the control panel, the old behaviour 
returns - the Startup Items only rarely execute (?). I can't figure out 
what triggers them to execute, but it only happens, on average, once 
every 10 or so restarts. (And, no, I am not going to restart 10 times 
just to check.)

But, I don't have total confidence in the Multiple Users feature. I have 
experienced quite a few crashes on a G4 tower, whilst debugging code, 
which never happened before Multiple Users was turned on. It is also nice 
not to have break up the startup sequence. If I restart (due to a crash 
or whatever) it is nice to be able to come back to a machine that is 
ready to go again, rather than entering a password and waiting again.

Anybody else have any ideas why Startup Items won't start? Surely I am 
not the only one to be using Startup Items with Multiple Users disabled?
