If you did a format of the target (or cloned) disk before trying to
duplicate the original startup disk, then I would guess the problem lies
with the source or startup disk. Could be a broken directory structure or
other kind of problems.

I might have missed it, but did you try running Disk Utility from the Mac OS
X system install CD on your source/startup disk?
