I concur with Alex. It's the impeller bearings. It's a common failure on 
gfx cards. It is usually caused by long continuous run times, which'd 
make sense if it was a server, although some just fail from old age.
If it is a GeForce2 the fan should be screwed on by 4 screws, you can 
undo them, lift it off and get it replaced - should be a standard size.
If it's a GeForce3/4 you may be in a heap o' trouble, the fans are 
custom made high performance units built into the heatsink, which is 
mounted on the top of the CPU and surrounding chips and is huge. It 
should still be under warranty with Apple if it's a GeForce3 though so 
it could be replaced.
