In all likelihood a bad card, but it might be worth it to see if you have USB Support 1.4.1 installed. You may also want to update to OS 9.2.2. I had a customer that wanted to add Firewire to a Lombard PB, and the card didn't get recognized under 9.04. An update to 9.2.2 solved the problem. It's free, too.
