Hi Cris, I have no probs with the V5, but I aint running QT 4.1.2, but 5.0.2, runs fine, no probs, alltough I've used 4.1.2 w/o probs aswell. IIRC the s/w version I am using with my V5 is the MacVoodoo 1.1.2 version, not the 1.1.3beta one. But if you're having screen glitches and such, looks like the card is overheating, or was exposed to a lot of heat for a while and could be permanent damaged, sorry to say that. You could try putting an extra fan over the card or flashing the card to a V4, so that only one GPU is used. If you're lucky the damaged GPU will be disabled and you can continue using the card as a V4. BTW I am using my V5 as a V4 so that Jaguar works with this card (still unaccellerated though).
