This was just mentioned recently, but I too have had problems with a Belkin USB card. In my case it was working fine in my 7500/200 but wouldn't allow booting into any OS after an Acard ATA-66 and Radeon were added to the mix (in my beige G3).

PCI card slot-swapping was a red herring. If the USB extensions are removed from either OS9 or OS X it boots.

The card itself doesn't say who makes it, and I borrowed an identical-looking card and got the same results. I borrowed a Keyspan and everything's fine again. Of course, all the cards have the same Opti chip but there's something else going one I guess.
