In OS 9, rebuilding the desktop would usually fix such a problem. IIRC, there is an equivalent to rebuilding the desktop in OS X.

But I hope that
someone will know, and will inform you.

On my machine, using 9.22,
I run Disk Warrior often.
Disk Warrior might help your problem,
and I would urge you to use it [often].
