I get lots and lots of failures using iTunes (on G4 400 AGP running most current Panther). I do fine with data CDs, though. I'm using an external firewire Yamaha 16-10-30. Most of my failures are when iTunes is checking the media. Once past that step, I almost never get a real failure. (I get plenty of failures to verify after, but the burn is invariably successful.)

I suspect I have a less than perfect cable--and any operation that takes a long time (i.e. checking media and verifying after the burn) is more susceptible to failure.

I read somewhere that there is a compatibility issue with certain brands of CDs and certain burners. I seem to have best luck with TDK, and worst success rate with Memorex--but that's strictly a very subjective impression.
