I have ahd the same issue for some time.... I have Mac and a PC sharing a DELL Trinitron tube over a USB KVM. If the Mac is not connected to the monitor (or the monitor is not powered on) I get the base video 640x480 - I then run a 'detect displays" to revert the res I want.
