The problem with this version of the UL2D card is that it wants to boot from bus 1, the external connector. So if you have an internal SCSI drive connected to the internal connector, chances are you WILL have booting issues. In general this card is a pain in the as* and I wouldn't recommend it to anyone who wants to boot from it. It's not supported by anyone. It's fine if you are just storing and accessing data, though and not using it as a boot drive. Cheap and fast.
Interesting... I just pulled this card from my Beige and put it in an 8500 with OS 9.1 on a SCSI Seagate Cheetah. This system (profiler) now sees the internal connector as bus 2, the external as bus 3 while the on-board SCSI is bus 0 and bus1 is the external (25 pin) on-board SCSI. This 8500 system seems to PREFER booting from the internal ATTO connector.
Go figure? I'll just leave it in there since my beige doesn't care too much for it.
