What kind/model of CD-ROM?  Is it the stock Apple one?

Might be worth it to replace it with a newer one.  My original Apple
supplied CD-ROM in my B&W was very freaking picky on booting from CD.  I
replaced it with a LiteOn Combo drive and it works /much/ better.

I found a graphic on Maxtor's site for jumper settings for that drive.

Generally, the top line is all you should need to look at.

I wonder if the cable is having issues as well.  It's rare, but not
unheard of.  Disconnect the IDE cable from the logic board and try booting
with a different cable in it's place.
