My MacDraw II always opens with a new window unless I double-click on an existing file. I did not experience the problems you describe in opening MacDraw in a variety of ways or in saving files.
A Get Info says that the suggested size for MDII is 900K and as I said before I've set it to 10,000K. I vaguely remember seeing the problem you're experiencing several years ago but don't remember the details. I don't know for sure but I may have set it to 10,000K then and that solved the problem. I find irony in the fact that a well written piece of software for Mac System 2.0 that followed Apple's programming guidelines - how long has it been... 20 years? - still works under 9.2.2 and possibly Classic mode in OSX, is still robust and useful.
