I checked the ROM version and it is 222 here is the system profiler

Also, the card must be flashed, it only has a DVI port, no VGA, so
could this be the issue.  I've cleaned out the cables and blasted them
and the ports with dust remover and made sure the cables were in
tight.  So far it seems all right, but the problem could manifest
itself again.  If it does, what should I do about another video card?
Any recommendations?
