Yes you need the fan and if it's excessively noisy it's likely that it's got 
bearings that are shot. I've found Mouser Electronics <http://www.mouser.com> 
to be a reliable source for fan replacements. You'll likely need to carefully 
disassemble the fan and housing, because you'll need to reuse the housing. Once 
disassembled, look for the OEM model number on the fan or search for it by 
dimensions.
