There is NO MYSTERY here. The MDD shipped with OS X v10.2.8 system
specific install disk set. Plus a MacOS 9.x Restore Disk set that
installs MacOS 9.x, after OS X, with a specialized MDD System ROM for
MacOS 9.x. If you don't have the full set of either, You must perform
an install a retail OS X 10.4.11, and use the Netboot disc set
downloaded, and installed from Apple to install MacOS 9.x, but after
install, BEFORE restart, remove the two Netboot Extensions to the
Disabled Extensions folder in the MacOS 9.x System file, then restart.

You may also have to remove a couple of the non-working ATI video
drivers from the MacOS 9.x System Folder to disable them.

Firmware level of the MDD for OS X must be at 4.2.8(f), and the ROM
for MacOS 9.x at netboot level, or equivalent. Firmware updates on the
MDD must be done usually from MacOS 9.x.

I have the original OS X Install Set, and the Restore Set that
installs MacOS 9.x on the MDD.series systams. I will make a post of
the CDR masters for both sets to mediafire, or yousendit etc.. that
anyone can download and burn media from Disk Utitity.

This silliness for obscuring, impeding the use of Mac hardware has got
to cease.

I will burn and mail a full MDD disk setn for the cost of the media
period for those who don't want/can't be bothered to learn how to do
it themselves. Enough, people!
