I am using NVDA and can read the BGT manual.
I first go to C:\Program Files (x86)\BGT and open BGT.chm.
This drops me in a tree view with headings like:
Introduction, Tutorials, References, Frequently asked questions, and the appendices at the top level of the tree view. Note that tutorials and references can both be expanded by pressing right arrow on them to get to sub items.

To read one of these sections press enter on it, then press F6.
You can get back to the treeview by pressing F6 again.
Visually the tree view is on the left and whenever you press enter on an item it updates a browser like view on the right. It is not obvious that you need to press F6 to get to this browser pane unless you've used the .chm file viewer before.
