That's exactly right. Sometimes, when in a hospital, I have been asked by a doctor or nurse, "Well, now, how are we feeling today?" I answer something like, "Well, I don't know how you? are feeling, but I'm fine." They are usually thrown for a loss on what to say next. It's just one of those phrases that is used, as you say, sort of like an adult talking down to a child. When I answer that way, I'm sometimes just messing around with them, and sometimes, returning their treatment which I don't like, because I'm not a child. I'm 55 years of age. (grin)

Being a Brit myself, the only explanation I can give you is that we
speak English badly lol.  It's a slang thing, tends to be used in
quite a patronising way, like an adult talking down to a child.
