This is something I've done, of a form. It was a fun little project, mostly because of the way ganglia makes everything so easy to parse.

Eventually the email got unpopular, so the project ended up being a web page that tied in with a hardware and ticket database that the admin could view and get a quick idea of what's down and what's being worked on.
