Replying to myself again. I managed to get what I wanted by exporting to latex and working through the instructions in the The LaTeX Companion for the index package and using dvips to generate my .ps file. This problem arises in poetry books where one likes to have an index of titles and and an index of first lines. Not the sort of stuff that techies tend to get excited about. But I must mention that John Smith, a former/latent physicist, was just named poet laureate of Prince Edward Island, so there is hope for all of you. Maybe in 1.4.
