I actually block all outbound traffic from staff segments and now I have
users
Who wish to use various SIP applications to chat.

Because I block all UDP and TCP I need a way to permit just SIP without
permitting all outbound UDP ports to anywhere.

There is no SIP application proxy built-in, but you can easily tunnel
inbound SIP traffic (UDP port 5060) to your PBX, or pass outbound
traffic through the firewall.
If both sides use NAT, it depends on the specific implementation you are
using; most of the times this will not pose a problem. Using VPN to
connect two phones works as well; we have been using this setup in our
production network for several months now without problems.
