Hi and welcome! I assume that it will be like last year, so I am just going to quote what Stephanie Taylor wrote on this mailing list last year. There are two option they can do the online form (you send the link to your parent and they can fill it out on a computer at work, home, etc) or the offline version where you can print it out for your parent to sign and then you load it back into your profile. When you submit the work for your first task your form will be reviewed at the same time the mentor is reviewing your work.
