Well, the non-domestic vets I trust do a fecal first and then decide what medication to use. This is called "shotgun medication." Not a good sign, but if he is the only one in your area see how it goes. This medication is not designed to stimulate appetites, it is designed to kill a range of parasites. His logic is roundabout, assuming this will kill the parasites and the appetite will then return.
He is right to assume all three will probably share parasites if that is the problem.
