What version of pcb do you use? I hit a similar bugs some (many?) months ago. Sometimes during the session there was no preview while moving objects or drawing tracks. There was no red cross drawn at the mouse after [ctrl-m] was pressed. I could reliably return to the correct behavior by toggling the "lock names" feature twice.
