I've seen effects like in these screenshots when magic_quotes_runtime was set to "on" in the PHP configuration (php.ini). Note the \" in the various edit fields. However, magic_quotes_runtime is not a setting that goes on or off all by itself. Are you or your hosting service playing with the server's settings on occasion?
