- I agree completely ... I web form would be ideal.
- privacy related, page 2, and most of the last page (married/divorced/single?? current and previous nationalities???) is useless,
why would you want to know that type of personal info???
- the pdf could also be made to include forms, since you have to print, fill-out with a pen and then re-scan or fax?
- using Microsoft Word in a survey about open-source/free software seems a bit insulting, also see http://www.gnu.org/philosophy/no-word-attachments.html
