OK, so I've successfully set up the attribute index in ANOTHER INSTANCE of MarkLogic we have running here and verified that the query:

max(for $i in cts:element-attribute-values(xs:QName ('InsurancePolicy'),xs:QName('index')) return xs:int($i))

is working correctly. It must be something related specifically to our install on the development machine where I was doing all of my work. I think we'll just start looking into settings install procedures followed for that instance in particular. Any ideas on what would cause this problem in the install would be appreciated, but for all intents and purposes, we can consider the context of the issue closed. Thanks for the help everyone!
