I'm new to ML & XQuery and am struggling to do a very simple pipeline

log file does not give much information and what I have noticed is that
my "pipeline action" works fine when loading a document through WebDav,
but when loading the document using the RecordLoader, the
action/pipeline is not executed at all (what I can see)

One thing you can do is enable the CPF trace events.  Go to
Group>Default>Diagnostics
Turn on trace events, and type in CPF to the event name box.
This will cause CPF to log all the state transitions as it goes,
which ought to give you plenty to go on.

One thing to pay attention to is whether the URI you are inserting
is actually in the scope of the domain.  The default domain is
a the directory domain "/": i.e. any URI beginning with / will work.
But that means that URIs that don't start with slash won't trigger
any activity.
