That's covered in the section on MVCC and updates. Documents aren't updated in place, which has the great benefit that each stand can keep its registered query cache unchanged even in the face of updates. Each stand can use timestamp tracking to eliminate deleted or past versions of modified documents from the results, while the new stand with the new version of the document includes the proper document version in the results.
I should point out, as a user of MarkLogic you never need to worry about this.
