Many thanks for your advice. I am sorry I am unable to reproduce the error as I 
have used the databases for various sample applications over the last few 
months. Removing from the "Trigger" database from the trigger setting and 
adding another database instead fixed the issue at hand

Somehow you ended up with two copies of the pipeline document in the Triggers 
database. If you can come up with a reproducible way to get into that error 
state, I'm sure support would like to hear about it.

If there isn't anything important in it, you might just clear the Triggers 
database and try again.
