I don't know if someone else already reported the bug...
There are couple of typos in the example web.xml given in the documentation.
http://jakarta.apache.org/tomcat/tomcat-4.0-doc/appdev/web.xml.txt
In line 78 and 82, </paramName> should be </param-name>
