Inspection of the message headers may give some clues. In particular, it 
should show *where* the duplication was happening: the duplicates will 
have the early Received: headers identical, and differ in later ones. The 
earliest different one will probably reveal the source of the problem. 

One possible cause is a mail server accepting a message for onward 
transmission but failing to get its acknowledgement back in time, so the 
preceding server thinks the transmission failed, and retries.
