mlx4: Qp range reservation

Prior to allocating a qp, one need to reserve an aligned range of qps.
The change is made to enable allocation of consecutive qps.

Diff from previous version:
- mlx4_bitmap_free() uses mlx4_bitmap_free_range() with range=1.
- Free qp range if failed to allocate qp.
