Hope this helps. Couple of things that may cause problems for you: It currently checks against an LDAP server. You can just take this code out if you want. It uses an extra column in the table, "added_by", to store who added it. Prevents abuse of the system. It also doesn't set the url to be active; but it does e-mail you a simple copy'n'paste command you can run on any host that has mysql installed on it.
